Under the Criminal Records Review Act, individuals working with children or vulnerable adults directly or potentially have unsupervised access to children or vulnerable adults must authorize a Criminal Record Check (CRC) for their employer or authorized organization. This authorization is submitted to the Criminal Records Review Program (CRRP) for review. 

 

The Act states under “employer” the following:  

 

“(j) a registered member who employs one or more individuals to assist the registered member in carrying on his or her occupation;” 

 

It is your responsibility to ensure all of your office staff have a CRC completed and cleared every 5 years.    

 

This is separate from the CRC required for CCHPBC registrants. The results of staff CRC are not to be sent to the College.

This is a manual process for employees – The Consent to a Criminal Record Check form must be downloaded, completed and mailed. 

 

Note: If an employee has completed a criminal record check through the Ministry of Justice they can complete a Sharing Form. This allows the applicant to share the results of a completed CRC within the last 5 years with another organization at no cost. 

 

The Application Form:

From the form, the following check boxes are required (See page 2 of the application for more information): 

  • Schedule Type “A” 
  • Works with “Children and Vulnerable Adults 
  • Part 1, Applicant Information 
  • Part 2 (b) – complete ALL of 2(b) using the office name and address.  Under ‘organization type’ check “other” and write in ‘health practitioner office. 
  • Part 3, not applicable 
  • Employee to sign and date the “Consent for Release of Information and Acknowledgments”
